Athletic Paperwork, Forms, & Physical Information

All athletes are required to complete athletic paperwork prior to being permitted to participate in any athletic activity.  The athletic department uses the platform Aktivate to collect paperwork.   This is a new platform beginning in May 2024.  A parent/guardian must first create an account and then register their student.  Please see the attachments below for directions on how to complete this paperwork, create an account, and how students can watch their mandatory videos each year.  You will need to upload your documents into Aktivate.  You will no longer be turning in your physical form to that Athletic Office.

FHSAA EL 2 - Revised April 2024 - MUST TAKE FORM WITH YOU TO YOUR SPORTS PHYSICAL

AdventHealth Free Physicals

AdventHealth and Volusia County Schools offers free sports physicals and ECG's once a year. Take advantage of the opportunity, register today. 

This free opportunity is available to Volusia County public school middle school and high school student - athletes, ROTC cadets, band participants, and firefighting academy students. Questions about the physicals can be directed to vcssportsphysicals@groups.volusia.k12.fl.us or to Mrs. Hahn at slhahn@volusia.k12.fl.us 

A student must be accompanied by a parent if they are under the age of 18 or bring along their EL 2 signed by the parent, the ECG form signed by the parent. These forms are available to download and print when you register. 

TRANSPORTATION AVAILABLE
Transportation is available for students if needed.  You must register for an appointment AND register with Mrs. Hahn for a seat on the bus. 

If you are unable to get to the physicals and wish for your child to get transportation to the Speedway for a physical, please register for a Saturday Morning - April 5, 2026 between 9:00am - 9:30am time slot and email Mrs. Hahn at slhahn@volusia.k12.fl.us and Mr. Hahn will drive a school van or bus to the physicals on Saturday morning departing UHS at 8:15am. PLEASE EMAIL MRS. HAHN at slhahn@volusia.k12.fl.us TO LET HER KNOW THAT YOU NEED TRANSPORTATION. Do not message her in SportsYou. If your child needs to ride the bus option on Saturday morning, they will be required to show the parent signed EL 2 and ECG form before being permitted to get on the bus.
