Athletic Paperwork, Forms, & Physical Information

All athletes are required to complete athletic paperwork prior to being permitted to participate in any athletic activity.  The athletic department uses the platform Final Forms to collect paperwork.  A parent/guardian must first create an account and then register their student.  Please see the attachments below for directions on how to complete this paperwork, create an account, and how students can watch their mandatory videos each year.
